Who is eligible for support?Pay It Forward Fund helps women who are struggling financially while being treated for breast and women’s cancers (cervical, ovarian, uterine) at the Humphrey Cancers Centers located at North Memorial, Unity, Mercy, & Monticello-Big Lake Hospitals. How can I apply for support? Eligible patients should tell their doctor, nurse, or social services that need financial assistance. These professionals can walk you through the process. How are the funds processed and received? After an application is approved, the patient will be contacted and a copy of their bill(s) will be requested. Checks will be mailed directly to the creditor, and a copy of the check will be mailed to the patient for their records. Please allow at least three weeks for checks to be processed. Which bills are covered? We pay essential living expenses including rent, mortgage, childcare, car payments, heat, electricity, phone, water and qualifying medical bills. How much money can a patient receive? Qualifying patients are eligible to receive up to $1,500 per year while they undergo treatment. How can I donate to the fund? You can make your donation online through our partner, North Memorial Foundation. Be sure to designate “Pay It Forward Fund” https://secure.groundspring.org/dn/index.php?aid=21843 We also accept donations by check or credit card. Checks made out to Pay It Forward Fund can be mailed to Pay It Forward Fund, 3300 Oakdale Avenue North, Robbinsdale, MN 55422. To donate by credit card, print and complete our PDF donation form and fax it to Kenna at 763-520-5657. What percentage of my donation will be used to help patients? Pay It Forward Fund is administered at no cost by our partner, the North Memorial Foundation, so 100% of your donation will be used to help a woman in need. Is my donation tax-deductible? Yes.
